Required before your child will be allowed to participate in Kinetic Kids programs, this form must be completed fully and signed by a medical doctor. Medical Release Forms are valid for 12 months unless your child has a surgical procedure or hospitalization within 12 months of the dated signature, at which point a new form must be completed and signed by a doctor. Please upload the signed medical release form to your child’s Playmetrics account before the start of programs. You may also bring this form to the first day of your child’s program or email to register@kinetickidstx.org. Other camp or school physical forms are not acceptable.
Required for participants with Down Syndrome, this form must be signed by a doctor and only needs to be updated if your doctor indicates a change in your child’s atlantoaxial instability. Please upload this form on your Playmetrics account or email to register@kinetickidstx.org.

Currently, there are no diagnosis restrictions or mobility limitations that eliminate a child from participation in our programs. There are certain classes that are more appropriate for some children based on level of mobility, diagnosis, and cognitive age-level. Our goal is to provide each child the best opportunity for success within the separate divisions set forth within programs. If there is a question on which division will maximize successful participation, the program director will determine the most appropriate division for that season. Kinetic Kids provides opportunities for children with special needs through sport and recreational activities. Although we pride ourselves on empowerment through inclusion, the physical safety of every child remains paramount. Kinetic Kids reserves the discretion to limit the participation of any child when the foreseeable risk is likely to result in bodily injury. Kinetic Kids is not liable and cannot control all photos taken since our programs are in a public, group setting. It is ultimately the parent/caregiver’s responsibility to ensure their child is not in photos if that is their intention. Any photos taken at Kinetic Kids, Inc. events become the property of Kinetic Kids, Inc. and parents/guardians agree by their child’s participation:
